Ischemic Stroke
“If you had a stroke- but could choose what type of stroke and where it was to occur in your brain- what type and where would you want it? Explain why and how your behavior might change. How can people overcome the detrimental effects of stroke? “
If I had a stroke but I could choose type of stroke and where it would occur I would choose Transient Ischemic Stroke. Because it is doesn't cause permanent damage. It is caused by a temporary decrease in blood supply to part of the brain, which may last minimum to five minutes. A blood clot often forms in arteries damaged by the buildup of plaques (atherosclerosis). It can also occur in the carotid artery as well as other arteries.
BEHAVIOURAL CHNAGES DUE TO STROKE
Stroke “induces behavioral and affective changes such as depression, anxiety, apathy that reflect the dysfunction of the whole system. Conversely, lesions in the system of instrumental functions induce signature syndromes (aphasia, anosognosia). At any delay from stroke, the diagnosis and treatment of mood and behavioral changes are a priority for clinicians and healthcare professionals to improve the quality of life of patients.
STROKE
A stroke basically is a condition that either occurs due to the blockade or bleeding of blood the vessels that interrupts the blood supply to brain from the CNS (Central Nervous System), which results in disturbed supply of oxygen to brain cells and ultimately the brain cells begin to die. Stroke is the second most cause of dementia and third leading cause of mortality. Normal cerebral blood flow (CBF) is approximately 50– 60 ml/100g/min. The reduction in CBF below 20 ml/100g/min results in an electrical silence and less than 10 ml/100g/min causes irreversible neuronal injury (Hakim M 1998, Jones TH 1981). For brain oxygen and glucose both are important and when the blood circulation is interrupted neuron transmission impulses are also disturbed which are responsible for the production of energy. Most of the strokes are of ischemic origin ascribable due to blockage of cerebral artery or arteries by blood clots which result in decreased cerebral perfusion. There is not direct treatment for neurologic injuries. “Thrombolytic canalization of occluded arteries may reduce the degree of injury to the brain if it is done before the process of infarction has been completed”. https://www.nejm.org/doi/full/10.1056/nejm199512143332401
 Types of Stroke
According to American Stroke Association which is a division of American Heart Association there are five types of Stroke which are:
1- Ischemic Stroke (clotting):
This occurs when a blood vessel supplying blood to the brain is disturbed. It accounts for 87 percent of all strokes.
2-Hemorrhagic Stroke (bleeding):
This type of stroke occurs due to the rupturing of weakened blood vessel. There are two types aneurysms and arterial venous malformations. The most common cause of hemorrhagic stroke is uncontrolled elevated blood pressure.
3-Transient Ischemic Attack (TIA):
Also called a “mini stroke,” This is a kind of warning stroke and should be taken seriously
4- Cryptogenic Stroke:
A stroke of unknown cause is basically called a “cryptogenic stroke.”
In overcoming the detrimental effects of Stroke there few studies held according to which “in a cohort of 110 patients, 56 (50.9%) were treated on evenings and weekends. The median imaging to reperfusion time in these patients was 111 min compared with 90 min during working hours (p=0.019). The proportion of patients with successful reperfusion (TICI 2b or 3) during the evenings and weekends was 82.1% compared with 76.7% during working hours (p=0.4). The proportion of patients with a 90-day favorable outcome was not significantly different in the two groups (64.3% in those treated during evenings and weekends vs 52.1% in working hours, p=0.2).”. https://jnis.bmj.com/content/6/10/729
