Corte uma foto em 125x250, eu vou usar essa

Abra um novo documento no photoshop de 500x250 e abra a foto nele.

Aperte duplique a camada (ctrl+j) da foto mais três vezes e arrume uma ao lado da outra.

Agora é só juntar as camadas, aplicar action e png. Sua square header está pronta.
