I think everyone knows my unpopular opinion that Sam would not wear lingerie at any age, especially as a teenager, but like...
everything he wears is a hand-me-down right down to his socks and underwear. it's got his shirt has got a stiff paint smear from when Dean was careless drawing a sigil and Sam wasn't even there for that hunt
most of his underwear are washed-gray except the pairs that accidentally got dyed splotchy blue the only time in living memory that Dean got brand new jeans. even his socks that are pairs have two different wear patterns and most of them are so big that he has to wear the heel all the way up on his calf
the lingerie might also be secondhand, but it's his alone. his nice special things and his secret. the underwear are pilled, but still satiny and they make him feel all aware and tingly. he doesn't get to wear the teddy very often but it makes him feel like people might want to see him, might look at him the way everyone looks at beautiful things: girls and paintings and fine jewelry. and for someone who feels so unseen and unheard all the time it's an indulgent fantasy
