the elevator one would be so good with fear wetting! maybe A and B get trapped in the elevator, and A has extreme anxiety about situations like this, and gets really claustrophobic... it could happen immediately. maybe they start panicking to B, and they can feel a slow, steady line of warmth running down the inside of their leg, but they don't even really care about that in the moment. then they look down and see the dark wet fabric of their pant leg, and the puddle on the floor around their shoe, and they start getting teary eyed from embarrassment. or, alternatively, they hold their pee... but they're stuck for a long time, and they get desperate. so when the elevator suddenly jerks to life and starts moving again, it shocks A enough that they can't hold it anymore. even as they cross their legs and try to stop themselves from wetting, pee hisses against their clenched thighs, and dribbles off of their butt and onto the floor.

for the too much coffee one, I'm imagining a person (A) who is infatuated with the barista (B) at their local coffee shop. they've been chatting a little, and one day B asks A if they can wait around til their shift is over. A agrees... but to stave off the boredom of waiting, they keep ordering big coffee drinks. by the time B is done working, A is uncomfortably full... they really need to pee. and eventually they do it in front of B, humiliating themselves by wetting their pants, and crying from embarrassment. B is taken aback, but they go into full reassurance mode... they still like A! they still think that they're cute, even if they wet themselves, just please don't cry!! meanwhile A's stream is still awkwardly happening in their pants.

woke up late for work, drank like four cups of coffee to wake up, and is now stuck in traffic. absolutely bursting to piss, but what are they going to do? maybe there's a cup or bottle, but no way that'll fit everything. they could get out and pee on the side of the road, but what if traffic starts moving again? no, no, they'll just have to hold it. bonus points if someone else is in the car to witness all of it

inhuman character in a human(oid?) body for the first time, has to learn about bodily functions. most of them are easy enough, but they take a while to figure out when they need to pee. a few accidents happen before they get the hang of it. maybe they even decide they like the feeling of it, and have to be told very explicitly (some punishment/negative reinforcement even) that they're not allowed to do that anymore

person who loses literally all bladder control/capacity when drunk either forgets their tolerance or is peer pressured to drink more than usual and has to deal with the consequences

fear wetting!! typically tough person gets in a situation a couple degrees scarier than what they usually deal with, and out it all comes. great especially if someone has to help them deal with the aftermath, comforting them and telling them it's okay, it's just your body, there's nothing wrong with you. maybe the comforter is a bit into it as well, and they fuck about it later :)

someone gets cursed to not physically be able to release their bladder unless under certain conditions, but when those conditions are met? immediate release, no matter when or where they are. could be something like not being able to piss unless at 100% capacity, or only being able to piss at a certain time of day, or even only while doing a specific other task or activity like sleeping

elevator omo... maybe the lift breaks down, maybe they're already desperate but the bathroom is on another floor, either way they wet themselves in front of whoever else is in the lift (be that one person or multiple)

however the desperation only gets worse and worse, character stops and starts crying, begging for relief "it hurts so bad"

this is for scenarios where the character really could have made it, if not for their own dumb choices

they were playing a video game and on a roll, and it didn’t seem worth it to pause. It would throw off their rhythm! Hours have passed and the game is still going great despite their constant shifting in their seat. After this fight they’ll pee. Honest.

or they were playing a video game without savepoints. Not a long one, but they already kinda needed to go when they started. Now they’re so close to the end but they’re also close to losing control…

at work and powering through a task that needed doing. Really there’s nothing stopping them from popping into the restroom, but if they get this done now they can go home early!

Technically it wouldn’t hurt the ingredients to sit on the counter for a second, but their hands are all eggy and to be honest they’re not sure they can hold it if they try to wash them. Pouring the milk into the bowl was already a close call.
