The ruby was made when Dream was young and experimental. When he was still figuring out the best use of his powers, how to work efficiently. Many other stones were made and stored, but the ruby was his and most powerful. Its ability to make dreams come true was meant to be used in budding species. To help give the universe a bit more life and levity. It held a concentrated amount of magic that could be easily focused, a bit like a magnifying glass compared to expending the effort needed to read small print with normal sight. At its normal level it held about 20-30% of Dream inside it. At its abnormal peak before its destruction it had as much as 50%.
Even when it was corrupted, it was still Dream’s in some way. It was a part of him and naturally fought being used by anyone but him to the point of causing him physical pain whenever it was used. When Dream found it after his sand, it basically lashed out at him in defense. Due to him not being fully recovered from imprisonment anyway it had significant impact. It was never meant to be more than a bit of concentrated power storage and would have broken at some point anyway once it drained Dream just because it wasn’t capable of holding that much power.
Dream never had the chance to examine it before it was destroyed and cleanse it. It might be gone, but what it returned to Dream is still tainted in a way. Trapped in a glitching loop of its last moments, ripping out his very being and restoring it. The loop happens at random and without warning. Dream still doesn’t know why it’s happening or how to fix it only that it’s worse than even what Burgess did to him.
