Where are the books with thin main who need to clear this 'I'm healthy' I can't remember.

A disease can be a factor that someone is fat or thin or maybe has follow diseases.

Castle in the air by Diana Wynne Jones has two fat characters and I hated it. They end with the bad guy...

The fatshaming was so useless. Why having to point out that the heroine is still thin after not seeing her for awhile? Why not 'he loves another women, why putting the fat nieces who giggle against the women who is beautiful and don't giggle'
