Two summers ago in Kansas at the National Women’s Studies Association Difference, a slide show was presented to the Lesbian caucus in which a series of myths about Lesbians was entertainingly debunked. The show was to be used in straight sex-education classrooms. One of the slides was a comic representation of the “myth” of butch-femme relationships with voiceover something like: “In the past, Lesbians copied heterosexual styles, calling themselves butch and femme, but they no longer do so.” I waited until the end to make my statement, but I sat there feeling that we were so anxious to clean up our lives for heterosexual acceptance that we were ready to force our own people into a denial of some deep parts of their lives. I know what a butch or femme woman would feel seeing this slide show, and I realized that the price for social or superficial feminist acceptance was too high. If we deny the subject of butch-femme relationships, we deny the women who lived them, and still do.

Christ comes from the Greek Khristós. Since many books of the Bible (and other gospels pre-Council of Nicea) were written in Greek, it was their way of translating the word messiah and it’s Jewish connotation into Greek.
Rather than his surname, it’s his title - like calling someone Queen or your majesty, instead of Elizabeth.
Greek was the lingua franca of the Mediterranean at the time, which is why some of the Gospels were written in Greek. St. Paul’s letters, which are some of the earliest texts in the modern New Testament, were written in Greek and use Christians (or rather the Greek for Christians). So I think it would be safe to assume that both came into use at the same time.
Disclaimer: This answer is what I remember from my History of the New Testament class ten years ago (a history, not theology, course). Someone may have better information.
