josuke could barely carry okuyasu's body when he was relatively uninjured. maybe he bulks up just in case he needs to carry okuyasu. lets say he doesnt even know why he suddenly NEEDS to work out his arms more, its just a need and if he doesnt do it, his stomach twists and his brain goes haywire.

what if something like dat happens again and josuke cant quickly carry okuyasu to safety? it could happen at any moment and josuke knows dat so as soon as he gets out da hospital, he trains his arms like a mad man. he needs to be stronger for okuyasu, if he cant help okuyasu without crazy diamond, whats josuke good for?

josuke could even test how much he can carry okuyasu, maybe dey wrestle and josuke tries to pick okuyasu up. he can only pick okuyasu up for a couple seconds so after, when josuke gets home, he trains harder. josuke might make it a game for em, he challenges okuyasu to see which one of dem can carry da other longer. josuke always wins but to him, its never long enough.

josuke offers piggyback rides, maybe he even tries to fireman carry okuyasu. in josukes mind, hes seein what r da most viable options if okuyasu were to nearly die again.

maybe koichi notices josukes off behavior before okuyasu, notin how josuke attempts to carry okuyasu around almost all da time. koichi brings it up and josuke just laughs, he tells koichi dats its just a game him and okuyasu play but koichi knows josukes lyin for some reason and he leaves it be.

okuyasu could point out how buff josuke has gotten in da arms, maybe he jokes about josuke usin him like a weight but josuke quickly snaps out "never, you're more important to me then that." and okuyasu wolfishly smiles back "no need to be so serious, josuke!"

his body dysmorphia worsens as he realizes he’s not strong enough to carry a body (not his fault tho, what 16 year old is) and he obsesses over being stronger > looking good because his devotion/protectiveness over Okuyasu overpowers his need to look “perfect” despite years striving for aestheticism (lean) over practicality (“bulky”).

Obviously it won’t like happen overnight but eventually he’s so traumatized and anxious over Okuyasu he starts (obsessively) working out / building strength.
