If I could here is what I'd do:
I'd make some goals for you with fun treats if you make them.
On days where you walk less than, say, 200 steps, you get a prize.
You get a prize for not changing your clothes -- with something special when you hit 10 days, 20 days, a month. (Extra special prizes for underwear not getting changes.)
Every day you go without showering, you get a prize.
Every day you consume your calorie goal, you get a prize.
When you hit weight and belly goals, big prizes.
And if you eat, let's say, exclusively McDonald's fast food for a month, no exceptions, big prize.
You'd get to choose the prizes too
