Canonically Dean is like... BAD at recognizing when someone isn’t who they say they are. He doesn’t consider that Sam might be possessed in s2 and absolutely takes at face value that he went darkside and killed someone in a fugue state. He also doesn’t recognize when Sam gets body swapped w some nerd kid in s5. Possessed!Bobby’s disowning of Sam (despite having yelled at Dean NOT to do that very thing literally one episode and probably only several hours prior in-universe) is disconcerting to Dean, but it doesn’t ring alarm bells. He confuses Meg for Ruby at first. He can’t recognize when Gadreel is playacting as Sam in s9. And even though he kinda gets the willies around Casifer and knows SOMETHING is up, he doesn’t think that he might not be Cas until it’s made absolutely clear. The only time he’s really ever correctly guessed that someone’s possessed is s1 with Azazel!John
