Because Satoru gets high as a result of this penetration, of this death. He is shown to be in a state of pleasure. And it’s specifically eyes rolled back. Body floating in the air. A dazed look and a giddy smile.

It’s ecstasy.

It’s orgasm.

And this becomes even clearer when you consider death as a euphemism for orgasm (see La petite mort) (see also).

Not to mention that Satoru, through this, reaches enlightenment. And what is enlightenment if not a higher form of orgasm—a bodily high that is considered the peak of pleasure, sometimes even referred to as an out-of-body experience. Except in Satoru's case, it does transcend to become spiritual and not merely in body.

Satoru experiences death. And he experiences the peak of pleasure in this death, a death that comes from penetrative violation. It becomes explicitly clear once it's all laid out: Satoru experiences pleasure in his rape.

Though, even considering these details, perhaps the biggest affirmation of it being sexual is simply... Satoru's own feelings about the moment.

What does this violation mean for Satoru/how is it seen in his eyes

Satoru, while pondering over the possibility of his loss during his battle with Sukuna, likens it to his encounter with Toji, expressing that he's feeling satisfaction at this.

But what adds to the sexual lens, there’s a specific word usage in the Japanese, that I was made aware of in this sukugo analysis (an incredible analysis that I highly recommend checking out btw!) (what I’m referencing is under the subheader Am I reaching? Is this reading intentional?)

The phrasing in Japanese of the latter panel, is:

今 彼を 満たしているのは― (Ima kare wo mitashiteiru no wa—)

In this specific moment, we have the use of the verb 満たす (mitasu), which is not only to satisfy, but to fill, in the literal sense. It's a word that, yes, at times has a sexual connotation.

This makes another translation of the phrase to be: "The one filling him now is—"

We're being told that Satoru is feeling the same satisfaction with Sukuna as he did with Toji, but more relevantly we're being told that he is getting filled by him in the same way.

And so therefore.

Gojo was killed—raped—by Toji. And he found himself filled—sexually fulfilled—by this to the point of orgasm.

Satoru’s search for this death and, therefore, rape again

But this doesn't stop here for Satoru, as what the previous pages tell us is that he's searching for the same thing with Sukuna that he was given by Toji.

Satoru's sense of fulfillment comes at the prospect of defeat. And what does defeat mean for him? Penetration of his Infinity. Violation of his personhood. Death. Rape.

With Sukuna, Satoru searches for the same satisfaction: for him to be filled again. He wants someone to best him in combat. He wants someone to take him and penetrate his Infinity. He wants to be violated again. He wants to die again. He wants to be raped again.

And Sukuna gives him this. He pierces his Infinity, pierces the entire world for his Infinity, and Satoru dies with a smile on his face.

Satoru dies sexually satisfied. Penetrated again. Violated again.

So, what can we conclude from all this?

Let's look back at what we've established.

Satoru, someone who is impenetrable, through forceful penetration, experiences such intense pleasure he reaches orgasm. Not only this, he also holds this moment as one of such sexual satisfaction, he desires to be penetrated and violated in the same way again (showing explicit signs of excitement at the prospect of it).

Well. There's no other way to say it.
