Ok so first I should say i really like the interpretation of resurrection that really emphasizes the toll that dying and coming back has on your humanity: dying and coming back should take something from you no matter how discomforting that idea is. You have to pay the piper somehow!

Because of this, I think they wait a really long time/ really prolong the original baby mev's first death/return. partially because they want to make sure the process is sound, but also because they can never really be sure what coming back will do to a subject until it's actually done. They try to really observe what they do and how they act to try and predict what will change, but ultimately the MOAT isn't really good at it, because this is the first time they've ever actually cared about what resurrection will do to a subject.

They want the Black Parade. Not some husks performing songs they don't care about.

So, before that first death, i think MEV is moody. bratty. belligerent, disrespectful. you name it. Even has a biting problem once they learn to restrain her arms when nurses come in to check their vitals. They're fiercely protective over other members of the Black Parade, and it often takes disciplining the other members in front of MEV to get them to comply. They're also probably terrified of death- their own and others- at this point. Like they could be much worse in their own rebellion, but they know the stakes could get much worse in turn. They want to leave with their lives, all of their lives. Nothing short of it. Because of this, the MOAT wouldn't tell them how trivial death is, since it seems to work at keeping them a mild nuisance at worst.

Until they separate MEV from her brother. Maybe someone high in the chain of command orders it, maybe it's some orderly with a mean streak decides to go rogue and fuck with the patients, maybe it's even a clerical error. But they're moved into different wings of the MOAT. And if she was a nuisance before, she's a straight up security risk now. Constant runs for the exit, slamming piano keys and screaming in the music room where they're meant to rehearse, aggravating other patients into rebellion. Even the occasional tumble with the soldiers who corral them all day. It escalates dramatically, but the MOAT is stubborn-- they don't like to admit when they're wrong. So the separation stays in place, and the rules are still firmly don't kill MEV yet.

It finally happens during a physical with Nurse Sylvia, bright-eyed and conniving young Nurse Sylvia. She's always been the only one who could placate MEV enough to actually get valid test results (no ripped out IVs, even if she did mumble and grumble the whole time). Even now, no one knows if MEV stole the scalpel, or if someone handed it to her just to see what she would do, but the moment Nurse Sylvia enters the room, she's pinned to the ground by MEV, and her poor eye gets nice and acquainted with MEV's violent thrashing and slicing. Nobody even understands the wails and animalistic noises MEV is making before a soldier, young and trigger-happy (maybe even has a crush on poor little nurse Sylvia), gets MEV's brains all over the wall behind them.
