Dr. Sarah Chen's navy blue trauma scrubs clung to her curves as she stepped out of her vehicle onto the rain-slicked asphalt of Highway 9. The rotating red and blue lights from the emergency fleet painted her face in alternating colors, highlighting her sharp cheekbones and full lips as she approached the chaotic scene. At 28, with eight years of trauma medicine experience behind her, she moved with a feline grace that belied the intensity of the situation, her athletic build evident even beneath the practical medical attire.
"Dr. Chen," Fire Captain Miller called out, his voice barely audible over the diesel rumble of engines and the hiss of hydraulic tools. "Female driver, approximately 30s, unconscious at the scene. Vitals are unstable—BP 80/40, pulse 130 and thready. Vehicle's unstable, we're concerned about secondary collapse."
Sarah nodded, her hazel eyes already scanning the crumpled silver sedan wedged against the guardrail. Steam rose from the ruptured radiator in ghostly plumes, mingling with the increasingly heavy rain that made her dark ponytail glisten under the emergency lights.
"Has anyone made patient contact?" she asked, pulling on a pair of nitrile gloves that hugged her slender fingers.
"Negative," Miller replied. "Door's jammed, roof's compromised. We're waiting for you to assess before we start cutting."
Sarah approached the driver's side with cautious steps, her steel-toed medical boots finding purchase on the glass-strewn pavement. Through the shattered window, she could see the patient slumped against the deployed airbag, her head lolling at an unnatural angle. Dark auburn hair, matted with blood and debris, obscured part of her face, but what was visible revealed striking features—high cheekbones, full lips, and long lashes that contrasted unsettlingly with her pale, unconscious state.
"Can you get me more light?" Sarah called to a nearby EMT, who immediately aimed a portable lamp through the broken window.
The additional illumination revealed a significant laceration on the patient's forehead, still oozing blood that had trickled down the side of her face. Sarah noted the way the seatbelt strap had pulled tight across the patient's chest, creating a visible deformity that suggested a possible collarbone fracture. Blood smeared the steering wheel where the patient's hands had rested at impact.
Sarah's gaze moved appreciatively over what she could see of the patient's body. The fitted athletic tank top had ridden up significantly from the impact, revealing a tantalizing expanse of defined abdominal muscles beneath—the pronounced six-pack with etched vertical lines that spoke of dedicated conditioning. Even in this compromised position, Sarah could see the patient's athletic build: toned arms showing faint vascularity, strong legs encased in compression workout pants that hugged every curve, the overall physique of someone who took fitness seriously.
"She's a fitness instructor," Miller mentioned, following Sarah's gaze. "ID in her wallet says Jessica Reeves, 32. Former competitive gymnast according to her social media."
Sarah nodded, her expression unchanged but her eyes darkening with interest. "Her conditioning could work in our favor if we can maintain perfusion. Muscle mass provides better physiological reserve, but it also means higher oxygen demand."
She continued her visual assessment, noting the patient's respiratory pattern—shallow, slightly irregular breaths that suggested possible thoracic injury. The patient's olive skin, normally sun-kissed and healthy, now appeared pale and waxy in the emergency lighting. Sarah could see the defined obliques tapering to a narrow waist where the tank top had shifted, the sculpted shoulders visible even through the fabric. The way the compression pants clung to muscular thighs and rounded buttocks was impossible to ignore.
"Let's get the Jaws of Life ready," Sarah said, stepping back from the vehicle. "We need to extract now. Her airway is patent for now, but with that head position, I'm concerned about cervical spine injury."
As the fire department moved into position, Sarah mentally prepared for the extrication process. Jessica Reeves was a young, healthy woman whose physical fitness might help her survive this—if they could get her out fast enough and prevent further injury during removal. The rain intensified, bouncing off the emergency vehicles and creating a slick sheen on the asphalt that made the extraction even more challenging.
