I want to preface that Mark's bread and butter is metanarrative. It's rife all across his body of work, and it's the kind of detail he puts thought into. If something was changed, it was changed on purpose, and I think with Iron Lung a lot of said changes are not done for brevity's sake.

In the movie, Filament Station was blown up. Ava says this outright, and also indicates that the porthole glass is from Filament.

In the game, Filament Station was rendered inhabitable because of a reactor failure.

Now, discrepancy like this forces us to look at this detail closely. Why is it different is a fantastic way to lead a thought. Now of course, it could simply be for the visual of Filament exploding, which is very cool, but I think there's more here.

Simon tells Ava, 'that wasn't supposed to happen'.

It's a really particular phrasing. 'I didn't mean for that to happen' could also work, but supposed to in a film with unavoidable destinies?

What was supposed to happen, then? We don't get details in the film. But we do have the game, and what happened in the game was a reactor sabotage. The point of difference is our active protagonist as opposed to a silent player character, so Simon is the focal point.

Ergo...

Simon was likely the one who sabotaged the Filament Station reactor to try and force a ceasefire, but instead of it just leaking radiation and forcing an evac, the reactor overloaded and exploded. Which was not 'supposed' to happen.

I think it's very interesting that the porthole glass, which cracks under the blood pressure, had 'not even a scratch on it'.
