I think all of us found the watch/clock motif a surprise. An interesting choice, given what we currently know about Lohen; choice that made me curious to the point of writing an analysis.
*I'm not an expert of pocket watches and the infos I gathered are fruit of a recent and quick research; same thing regarding Lohen and his lore, since I'm writing this before his release and I avoided leaks. For this reason, given that I don't have enough materials to write a proper analysis, this is more like a draft and anything is subject of intepretation and change. Really, this is pretty rushed since I wanted to post it before Lohen's banner, so there're probably some mistakes.*
As first thing I want to analyze the pocket watch's exterior.
Lohen's watch has a striking resemblance to a "Lèpine", a type of pocket watch characterized by its lacking of a metal cover and for having the pendant and the winding crown at the "12 o'clock" position. The decorations on each cardinal points (12 – 3- 6 – 9) is strictly part of Lohen’s design (some part of it are present also as the harms' points) and that make me think that is actually a crest of some sort, most probably his family's.
Even the colours resemble Lohen’s, with the only difference the two yellow spots (I’ll get back on them later). Beside some numbers there are suns (between the XII and I, a smaller one between VIII and IX) moons (smaller one between V and VI, a bigger one between X and XI) and stars in the hand harbor (two at the side of another sun, one mirrors the little moon on the other of a little diamond decoration).
Just from this first look is clear that this is custom made and luxurious item.
The numbers are written in the Roman format, and the watch has two subdials. Usually the subdials are used to counts seconds, as a part of a chronometer (if I understood it right), to shows the current day, even to shows the phases of the Moon. Of course, being a game is unlikely that those are the uses of th subdials in question.
If we take a closer look, we can see that the one on the viewer right resemble a roulette’s wheel and its arm is aleady pointing to something (I doubt is a number…). The second subdial, smaller than the other, cover nearly all the VII and VIII numbers on the lower left; this is one is more peculiar, since it both resemble the pocket watch itself and a type of target.
Now, let's take a look at what I think is the inside.
Beside the fact that this is too similar to a cartridge and just before we saw Lohen using a gunlock (?), in this image we can see that this stricly resemble both subdials: the roulette design of the right subdial, the target-like section of the left subdial and even the numbers in the "weapons slots".
Speaking of which, I saw many saying that weapons showed on this "clock" are the one Lohen uses in the trailer, but I disagree. If you look closely, you can see a yellow section, in the same position of the ones found under the V number on the exterior dial and on the target-like subdial.
During the trailer, the arms of the clock run back and forth from a weapon to another at an extreme high speed (the moment right before the arm stops on a weapon, sparks fly all over). And everytime the weapon is on the yellow spots, that is the exact one Lohen use right after or before the "scene"choice scene":

Why the chosen/designed weapon is on yellow? Sincerely, I don't know. The only thing I can think of is the symbolic meanings of the colour, but none of them can really fit here (primaly, the cowardice and betrayal). Maybe it symbolize Lohen's joy?
Meanwhile, the rifle and the "apple target" are on a "void" spot:
This because the rifle is not used by Lohen but is the only other kind of weapon present at the moment of the "confrontation", meanwhile the apple, if it represents the memebers and participants of the criminal auction, is in the void spot because is not Lohen (it also has a dart sticking from it). And, well, they have no say in this geme of gambling.
Beside the weapons-slots, another striking part of the watch's inside is this:
The second arm-thing is the same roulette's wheel in the first subdial. So, is like a never-ending game of choice and gambling: depending on the enemies'weapon the clock choose what Lohen should use, but at the same time there this other game going on, maybe something inner.
Maybe connected to the way the weapon-clock changed into a bullets chamber, with only a bullet inside:

Or, since the shot right after this take us back at Lohen interrogating that man, maybe the mini roulette wheel represents that there are two more gambling in progress?
During the fighting, the weapon-clock sequence is lighten by red and blue hues, most of the time together just like the image above. But there are moments when one colour is present:
I don't know what this means, but I find it really strange and something since for the umpteenth time Lohen is associated with red and blue. Why are they so similar to the red and blue kuuvahki? Or is the red a "symbolism" for the abyss and the blue to Mondstadt, like the old age concept of red=bad/rage/hate (but also love) and blue=good/calm/peace (but also sadness)? Or there's something more?
Another quick thing: the moment when Lohen starts "fighting" (because that wasn't a fight but I don't know how else call it) and exit the box, the minute hand overlap the crest symbol at 12 o'clock:
You can see the moment when all is decided.
In conclusion, I don't have a conclusion.
I mean, this is all speculation from me but it's clear to everyone that there's so much more hidden in the trailer and the clock motif is just one thing.
In my opinion, the pocket-watch is another portrayal of Lohen. We already know that he define himself as an arrow, but with this trailer, I think that that arrow is also the clock hands: they do their job, they tells the time but they are part of a mechanism and have no free choice (ofc they don't, but you get me where I'm going). In Lohen's case, is almost like he seems a free spirit, maybe even someone who doesn't care of the consequences, but in reality, deep down, he crave freedom from something he knows or perhaps doesn't know.
