The first time i watched the movie it was in italian and idk if that's something generally known or if anyone ever discussed it at all but in the ita version the last words have been dubbed, (they're not in the subs tho, which is strange but oh well...). Gojo says:
"Suguru, caro amico mio..."
"Suguru, my dear friend..."
Now, a few premises,
Italian dubs and adaptations have a history of chensorship
Doesn't really explain geto's reaction, unless it's bc he realized gojo only ever thought of him as a dear friend and never really hated him
What i consider canon is what get written down in the manga, everything else is mmmm not so canon
But regardless, as much as i would love to hear I'll be lonely without you or I love you, i still think in truth it's something along the line of "you're my best friend", or "you're my dear friend".
Now, on the meaning and the translation:
Caro amico mio it's not the same as mio caro amico: 'mio caro amico' is what translates from 'my dear friend', the actual litteral translation from the dubbed line in the italian version of JJK0 it's something like 'dear friend of mine' (but that was uselessly verbose imo so i picked the simple version, the dots kinda make up for what gets lost).
so,
caro amico mio,
mio caro amico,
amico mio caro,
...
It's always those three words, caro means 'dear' but when you put it at the beginning of the phrase it stresses the meaning of dear, giving it a sweeter and and a tad less colloquial nuance.
