The eighth level of the game, the fourth in the mansion's sewers, and the second of the game's secret levels. The player must overcome pistol-bearing Mole Fish, charging Monsters and Fireballers. The level is a slaughter map filled with a massive horde of enemies that is most easily dealt with by sprinting to the left of the arena to collect the chaingun and ammo and dealing with the monsters as they cross the map.

The sixth level of the game and the final to be set in the mansion's sewers, also features the game's first boss, the Rat King Baby. The arena is composed of a large central room with columns that can block the boss' attacks and side corridors surrounding the arena with health and ammo packs.

The Rat King is a large immobile horror that spits fireballs in the middle of the room supported by two tentacles with rocket launchers that can slowly follow and fire at the player. The boss is invincible to harm until both tentacles are killed, causing it to briefly enter a vulnerable state indicated by its red eyes opening. After a few seconds the boss descends into the water and then emerges creating a damaging wave that can be avoided by hiding in the side corridors. This process repeats until either the boss or the player is dead.
