He has always been very protective of Sam ever since he helped him escape the safe room at Bobby's as part of Heaven's plan to trick him into starting the Apocalypse. Between that and breaking the wall in his mind I feel like Cas's guilt over his own actions and sense that he owes him has developed into an extreme drive to keep Sam safe, not unlike Dean's.

Speaking of... Dean calling Cas off after a mere headbutt? Very out of character.

In fact, Dean should have been right there, punching the guys teeth in. Cas may be so protective of Sam he can fight off Lucifer's hold on him to save him, but that still is nothing compared to how Dean is.

The only thing that makes sense about it in this episode is that Dean is holding himself and Cas back because he doesn't want to scare Mary, or make her ashamed of how violent and dangerous her sons turned out to be.

I feel like this is confirmed by the surprise on Dean's face when Mary tells Cas to hurt the vet to get more info.
