I think Tsunade absolutely hated Kotetsu’s guts when she first took up being Hokage. His attitude rubbed her wrong and she probably assumed he was gonna end up being a bigger issue than he actually turned out to be. He won a spot in her heart with his ridiculous office shenanigans and not once questioning her drinking on the job.
Relatedly, since Kotetsu is capable of keeping a secret, I think she rewards him with a shot of her expensive, yummy sake whenever he does a good enough job and thoroughly impresses her.
Tsunade has a soft spot for Naruto, everyone knows this, and I think it genuinely puts a smile on her face when she catches the rare happening of Kotetsu and Naruto bickering and bothering each other like siblings.
Whenever Kotetsu and Izumo are having a lover’s quarrel and they’re doing a bad job of hiding it, she tells them both to just kiss and make up already because their stank attitudes are mucking up everyone else’s vibe. Genma doesn’t act right, Kakashi makes him self scarce 4x faster than usual, Iruka gets a little bit down— it’s an office wide thing. If it goes on for more than a few days, Tsunade usually arranges for them to get gift baskets from “each other” and designate herself as the common enemy just so they’ll get over their fight faster. The things she does to see these two in love.
Although Kotetsu isn’t Tsunade’s #1 errand boy, and he’d never want to be, he does a good job of remembering what type of snacks she likes from the convenience store. Kotetsu made the mistake of coming back without a snack for her once and he’s never made that same mistake again.

Tsunade's birth
There once was a beautiful maiden from an island of Kyushu, where the sea winds smelled of salt and pine, a place far away from the Fire Country. She was very kind, helpful, and clever. She had a mother who was a sorceress, and a father who was a daimyo who served his land well.
When Kaito was small, I used to tell him stories. He was always sickly in the wake of winter. I still see him recovering in my bed, glaring at me with his giant, black eyes, asking me to tell him a story. He said it would help, so I had no choice but to oblige peacefully.
I used to tell him stories about ghosts, about brave warriors, but Kaito liked one story the best. It was the story of Tsunade-hime.
One evening, when the lamps were low and the house smelled of Mito's herbs, Kaito crawled into my lap. 'Please, dad, tell me the story about Tsunade. I'm dying.' He said that every night. And I repeated the story again.
One day, in the middle of November, when leaves were turning maroon and yellow, Tsunade-hime wandered a deep forest, gathering firewood for an old, gentle woman. The old woman was sick and didn't have wood for a fire to keep her small house warm. A man sluggishly approached her and said that, since she was so kind, he would teach her his ability to heal and fight.
He listened with his whole body, like children do. His eyes widened when a revelation was coming, because he knew them all, but still acted surprised.
'Dad, if a slug asked me to learn magic, I'd say no. I'd say I'm too busy.' Kaito was so earnest I laughed. But he would have cried if I did, so I didn't.
Tsunade-hime was taught how to heal the poor, destroy thieves, and bad people in that very forest of Kyushu island, where birds chirped even at night and where it was always drizzling. When her teaching was finished, the man revealed he had been a slug the whole time. She was so surprised she tripped and fell into the embrace of a man named Jiraiya.
Later that night, he asked me if Tsunade ever got tired of saving people. I said I don't know, but that she probably did. He said she probably cried sometimes, and that he wouldn't want to be the strong one all the time. He was so small back then.
Tsunade-hime and Jiraiya were travelling the world, saving poor, fighting the wicked thieves. One splendid night, Tsunade was bitten by a venomous serpent. And her page had to travel to India for an antidote.
'Have you ever been to India?' Kaito asked. I laughed and said it was a fictional land. He looked at me with a disappointed expression, like I just ate his favorite snack. 'And have you ever been to Kyushu?' I smiled at him, and said yes. He was thrilled.
And today, his daughter was born. I knew she would be a girl. A tiny thing, warm and loud, furious at world already, with fat arms. Her fists are so small, but they look like they could break my nose in half.
Kaito stood beside me, pale, trembling, the same way he trembled when he got his injection and screamed, 'DAD DO YOU EVEN KNOW WHAT I'VE JUST BEEN THROUGH?' He hasn't changed, he is still dramatic at least twice a week.
He looked at his newborn daughter like she was a miracle he didn't deserve. I said that he, in fact, does deserve her when she started to screech. Her screams echoed through the gray hospital corridors that smelled like alcohol.
When I asked him about her name, he asked me if I remember the story about the girl who healed everyone.
He said that he wanted her to be named after her. Because she reminded him of me. And of Mito. And of the village.
Mito smiled and said that the name felt right. She said Tsunade-hime carried magic meant to protect others, just like Uzumaki do. Just like she does.
And I thought of that story, of the girl who held the world together, of the quiet strength she carried. I looked at my first granddaughter, my Tsunade-hime, and I felt like something inside of me loosened. I won't disclose if I cried.
And when she opened her eyes again and started to scream, bright and alive, I thought that she will hold everything together. That she is the rope, the hand, the name.
I whispered the name to her, like blessing, "Tsunade." And she cried some more, as if she already knew what the world would want from her one day and what the world will surely take.

The way they bond is over spite.
Karin only talks to Tsunade because she thinks she annoys her and since she had been a friend (or more?) Of Orochimaru. Not only related by their clan connections but also from the person she views as the closest thing to her parent she begrudgingly tolerates her.
Tsunade can read this like a book and uses it to push Karin. For better and worse. For one Karin is able to keep up with Tsunades evergy. Being so loud, bombastic, and unapologeticlly themselves they both can let lose without risk of causing real harm. Due to this Tsunade does try to annoy Karin back and shows her personality more unrestrained because one she knows it will egg Karin on, and two because she knows Karin can actually handle it unlike others.

Operation: Tiny Kunoichi
Hashirama had been ambushed. Not by enemy shinobi, not by political rivals. He had been ambushed by his lovely wife Mito.
"Hashirama, you're in charge of her today." "All day?" "All day. It will be fine."
Hashirama knew it would not be fine. He had to retrieve an old scroll. Not just any scroll, a very important Senju scroll he had stored in the old Senju compound, the one no one used anymore, because it was half-collapsed, drafty, and full of memories he didn't want to face. It was also quite far from Konoha, behind woods and swamps. He knew how much Tsunade hated walks. He tried to take her to the river once and she told him she thought he was a serious person.
He also wanted to visit Tanzaku-gai on his way back. But his best laid plans were ruined by Mito's plan. It wasn't that he didn't like to spend time with Tsunade, he loved her and he spoiled her, but the scroll was important for his next jutsu.
But he couldn't leave Tsunade alone. So he improvised. He crouched in front of Tsunade, grabbed an old cloth, and tied it around her head like a forehead protector. He drew a Konoha symbol on the front. He gave her a paper kunai. He found a tiny west, and dusted it of.
"Tsuna-hime, I have a mission for you, for us. Only you can help me," he looked at her with the seriousness of a Hokage about to brief an elite squad.
"I need a very brave ninja who will accompany me to the old Senju compound and who can keep secrets. And I think you're perfect for that."
