Narrator uses hand cream and lip balm cause he hates having those dry and chipped. It's one of the things that tyler makes fun of him for, but rator's reliance on these products is so strong that he does not falter at the teasing.
Actually tyler, very deep down, likes how smooth his skin feels, and overall how much care he takes of himself, but this appreciation is in strong contrast with his rejection of social norms and capitalism.
Therefore when the narrator gradually lets himself go, tossing away his composed and clean image, tyler feels triumphant knowing that he's dragging him to that ultimate low he's supposed to hit.
Except that he can't help but miss when they'd accidentally touch hands on the bus, when he met his smooth olive skin instead of the coarseness that already belonged to him.
