So the exterminations have only been around for 7 years? Given there's cell phones in Hell, we know that the show is set somewhere in the 21st century. Before then, sinners couldn't be killed (unless they were the overlords Alastor targeted)
So where is everybody?
No seriously, let's go with the 100 Billion total humans ever figure. Let's say further that only a small portion, 5% were bad enough to go to Hell. That's still 5 billion people.
Those people would include Roman emperors, Ghengis Khan and his conquerors, British slave traders, Hitler and the nazis, etc
So where are they? Why haven't we seen Stalin or anybody else pop up with a cult of personality? Or seen their influence on Hell like through Roman military traditions or seen swastikas plastered on buildings. You're telling me Alastor and the exterminators managed to get all of them? As in, even the people inspired by them?
Millions of people joined the crusades and the nazis and other large movements full of genocide, rape, and other sin. Enough to make large communities and have cultural transmission to older communities when they get down to Hell. If Pentious went to Hell for turning a blind eye to murder, where does that leave the literal tens of millions who in the past century alone have turned blind eyes to genocide?
This is turning into a ramble, but are you guys seeing my point? There would be so much conflict alone from having communists vs nazis squaring off, there should be evidence of endless chaos as eras clash and fight for their own ideas to stay alive (or undead in this case), alliances forged between peoples with similar ideologies. Hell shouldn't be anarchy, it should be bloody civil war
Show me former nazi sympathizers coming to the Hotel. Show me Roman soldiers weighed down by 2000 years of guilt. Show me slave traders arguing with Charlie over the morality of the slave trade. Show me crusader's spitting in the face of the devil's daughter. Show me what redemption means for those who committed the worst of the worst sins out of malice and opportunity, not out of cowardice and circumstance. Show me how you reform a society made of the true worst, not the street level murderer, but the designers of genocide and empires
