so chidori, after years of not fearing death, became scared of dying for the first time in her life after meeting junpei. despite feeling like she didn't belong in this world, junpei became her reason to stay in it. she was in pain in more ways than one, literal thanks to medea acting out, and emotional thanks to her growing feelings for him.

despite this newly regained fear of dying, in what were supposed to be junpeis final moments, chidori used medea to emit her own life source. not only so junpei could live, but because she wanted to be with him forever in some kind of way.

also, hermes and medea merged together as a part of junpei's evolved persona, so she will always be fighting alongside him. she may no longer be in the physical realm, but she is with him no matter what. just as she wished it.

holy crap thats easily the most romantic thing atlus has ever put in a persona game.
