As the real, human brother is quite dead, Dante ends up killing the demon, much to the sister's distress, as she believed her brother could be saved.

Dante does not get his promised payment for the job, but does get a line on what's going on with the new plastic surgery joint in town, which he begins to investigate seriously.

Dante finds and interviews more customers of the shop, some possessed by demons, some not, all very different in terms of both looks and personality than they were before.

Middle

Eventually, he makes it to the shop itself, which is run by a surgery demon who not only claims to like humans, but to also be a big admirer of Dante himself.

In his mind, the things he's doing are good. He's taking out the garbage, picking up the unworthy and worthless off the streets and recycling them to improve those humans with meaningful value.

He is more than willing to operate not just on the body, but on the soul, and should his patients come in sad and unhappy with their everything, fixing the problem is as easy as finding someone else to be them instead. (This is what happened with the brother)

The surgery demon has heard all about how unhappy Dante is with himself, and says it would be an honor if Dante would please lie down on his operating table, so he can cut that pain out.

He also reveals an entire selection of living humans, some in pieces, some not, for Dante to select from, should he wish for a new face.

Dante is tempted, deep down inside, but refuses, because: 1) He is actually capable of feeling human care and empathy, unlike the surgery demon, who thinks he does, but can not. Dante understands what is happening here is wrong. 2) He's tried running away and becoming a different person before, and it didn't work. 3) Dante feels obligated to carry his guilt as a way of paying for the wrongs he feels he's done.

End

The surgery demon understands none of this, and feels terribly betrayed, not just at Dante's refusal, but that he's so determined to kill him and ruin all the "good" he's doing for the rest of the world/humankind.

He states Dante's refusal to change is the reason no one likes him, that his inability, or even tendency to avoid or refuse aid from those who would help him is a weakness of which he should be ashamed.

Dante kills the surgery demon, then frees the captured humans who can be freed (They all run in terror,) and puts down the disassembled pieces of people who cannot be saved.

Lady comes into the shop later, where Dante is on the tag end of a drinking binge, staring into the bottom of a glass while listening to the radio, now tuned to an official news channel, where the announcers are reporting on the sad loss of a local clinic to the depredations of a maniac, alongside the sudden increase in homeless and vagrants wondering the area, most of whom are ranting about demons.

Lady asks what the problem is.

Dante: "Who says there's a problem?"

Dante still refrains from fully finishing his drink,continuing to stare into the bottom instead.

Lady pours herself a cup, calls bullshit, and demands he tell her what the problem is.

Note here all the mirror in the shop were taken or broken long ago, but that Dante still finds places to get caught by his own reflection anyway. It's an image he not only can't, but won't escape.

The reflection in the bottom of the cup stares back at him with a sense of accusation that reminds him a lot of Virgil.
