Oral Submucous Fibrosis (OSMF) Excision and Reconstruction with Nasolabial Flap This gentleman who has been chewing gutkha paan for over 10 years noticed a gradual decrease in his mouth opening with very minimal mouth opening at the present time. He also noticed a hardening of his cheeks. Diagnosed with oral submucous fibrosis, he had undergone multiple interventions elsewhere including skin grafts, which had not resulted in resolution of his symptoms. A local surgeon had referred him to our hospital. Treatment planning was explained to him and he consented to surgery. He underwent bronchoscopic intubation because of the inability to open his mouth. Incisions were marked along natural skin creases to minimize scar formation. The fibrous bands in his cheeks were then excised followed by inferiorly based nasolabial flaps. Tunneling was done and the flaps were rotated and sutured intraorally in the buccal region. Extreme care was taken to ensure that facial artery vascularity was not compromised. There was approximately 4 cm mouth opening postoperatively.
