I think the return to the sex hospital is to show the reconditioning of the clerk. We saw at the end of the Latam leg of the tour that Gerard had seemingly been reconditioned into accepting the Long Live jacket and becoming the most exalted vocalist. During this tour, it seems like he's more willing to follow instructions like, during the London night 1 show, when the note before Cancer said to "slap him" (in reference to the clerk) and they did it, seemingly without a second thought. A similar moment happened during the night 2 show when the secretary handed them a note that said "stand still" before she slapped them twice.
In contrast, the clerk has been hesitant to do anything to hurt Gerard since the beginning of this leg because they're friends AND he hasn't been fully reconditioned yet. The show in Florence is showing us the continued reconditioning of the clerk because he would never have tried to hit Gerard during the Latam tour like he did on wednesday. And even after he almost did it, he looked horrified that it was something he even considered for a second.
