Thinking about med school Michael who hasn't yet gone by robby to anyone. Hasn't yet thought to change/modify his surname because he hasn't experienced all that shit yet.
Sure he occasionally gets people going "robina-what? That's a funny name" when he introduces himself. But it's not often.
He hasn't yet introduced himself, "hi I'm Dr Robinavitch, I'll be looking after you today", over and over again. Each time a lottery on what reaction he's going to get. Sometimes it's fine. But sometimes it's not. Maybe strange remark or a face pulled. Or a hesitation that sits between them for the rest of the treatment. Or outright aggression or violence, which after the first few times made him scared to come back to work.
There's a time where he gives up being "Dr Robinavitch" and I'm just thinking about the dinky med student about to set foot in a choas of the er and have to learn that for the first time.
