Common Dosage Forms:
Tablets: 400 mg, 600 mg, 800 mg
*Also available in nonprescription formulations containing 200 mg ibuprofen and in various nonprescription liquid and chewable forms for use in children.
FDA Indications/Dosages:
Mild to moderate pain and primary dysmenorrhea: 400 mg every 4-6 hours.
Rheumatoid arthritis and osteoarthritis, including flare-ups of chronic disease: 1200 mg to 3200 mg daily in 3-4 divided doses.
Juvenile arthritis: 30-40 mg/kg/day in 3-4 divided doses.
Monitor: LFT, RFT
Pharmacology/Pharmacokinetics: Ibuprofen produces a reversible inhibition of cyclooxygenase which results in a decreased synthesis of prostaglandins. Prostaglandins sensitize pain receptors resulting in a lower pain threshold. By inhibiting their production, ibuprofen raises the pain threshold so fewer pain impulses reach the CNS. Prostaglandins are involved in setting the normal set point in the thermoregulatory center of the hypothalamus. By inhibiting their production, ibuprofen resets the normal set point and increases heat loss from the body through cutaneous blood flow and sweating. Prostaglandins are also involved in vasodilation and plasma exudate formation at inflammation sites. Again, inhibiting their production, ibuprofen can decrease inflammation. Peak plasma levels occur in two hours. Elimination occurs in the urine. Serum half-life is 2 hours.
Drug Interactions: Aspirin increase plasma clearance and probenecid decreases plasma clearance. May increase toxicity of lithium. May decrease the effects of beta-adrenergic blockers. May decrease the actions of loop diuretics.
Contraindications/Precautions: Contraindicated in patients hypersensitive to nonsteroidal anti-inflammatory agents. Not recommended in pregnancy due to the known effects of this class of drugs on the human fetal cardiovascular system (closure of the ductus arteriosis). Use with caution in patients with renal and liver impairment, hypertension, fluid retention, heart failure, bleeding disorders or a peptic ulcer, and in the elderly. Patients who develop visual complaints while taking ibuprofen should have periodic ophthalmic evaluations. NSAIDS MAY CAUSE AN INCREASED RISK OF SERIOUS CARDIOVASCULAR THROMBOTIC EVENTS, MYOCARDIAL INFARCTION, AND STROKE. NSAIDS HAVE BEEN INVOLVED IN RARE CASES OF GI BLEEDING INCIDENTS. No Pregnancy Category Established.
Adverse Effects: Indigestion, constipation, abdominal cramps, flatulence, dizziness, RASH, pruritus, heartburn, nausea, diarrhea, tinnitus, and anorexia.
Patient Consultation:
Compliance with therapy is essential, may require up to 2 weeks to see noticeable improvement in arthritic conditions.
May take with food or milk if GI upset occurs.
Concurrent ingestion of alcohol will increase ulcerogenic effect.
Avoid aspirin or aspirin-containing products without first consulting a physician.
Store in a cool, dry place away from sunlight and children.
Notify a physician if adverse effects are bothersome or if a rash develops.
If a dose is missed take it as soon as possible. If it is closer to the time of your next dose than the dose you missed, skip the missed dose, and return to your normal dosing schedule. Do not double doses.
