in the movie he remembers her quirks of reading the same book over & over, and how much he misses her beside him; it never delves into the ugliness of their relationship breaking down..that is unless you count the moments he is hallucinating the fact that Sammy Jankins had a wife who tested his ability to remember, which would make it a bit creepier—I guess the Nolans were trying to signal how they felt about serial killers who get away with murder(?) ..but I'm not completely certain given how many shows have been based off this exact premise of righteous killings & of righting wrongs—personally I'm not convinced that was the purpose of the movie bc the central moment is when the mc (Lenny) says he used to think the spark of familiarity in Jankins' eyes was a sign he remembered Lenny, but he now recognizes it is just s/t we all do when we want to fit in without admitting to the ugly truth of our condition, so we pretend we recognize ppl we have never met before or have no recollection of ever being introduced to.
It wasn't about faking, he explicitly states it, he even shows it when he can't remember if he's met them or paid rent & the guy at the frontdesk is like you're paying for two suites. He also repeatedly denies ever saying Sammy Jankins was faking, out of remorse over the case; and to back it up, Natalie keeps asking him if he really doesn't remember her which is a sign that he too has the spark of recognition despite not remembering.
And although there are points I have my doubts like how did he know to drive to the rundown & discount motels except through consistent repeated exposure(?), but maybe we're just not shown how he struggles to get there so the movie isn't 6hrs long. He was angry with Teddy & suspicious of his character, and leading up to the final confrontation Lenny is asking is that true and as he searches his pockets, he finds the photograph with the words: Don't trust Teddy's lies. Both Teddy & Natalie separately inform Lenny how she can say & do anything and he'd be none the wiser or other ppl could take advantage of his amnesia. The Discount Motel front man admitted to the same thing, Lenny is being had & redirected by ppl constantly so it's no wonder he can't trust much of anything.
The thing is he can't form new memories so I think Sammy Jankins really did have a wife who died of the overdose & Teddy being the perv who called him on the phone, got every little detail from Lenny who regularly spoke to s/o on the phone about Sammy Jankins. I think this whole thing is about regretting he wasn't more understanding of Sammy Jankins' plight & now that the chairs have changed, Lenny's suffering is twofold: He can never relish the revenge he wanted for the killers, nor can he go back in time to change his decision on the insurance claim to save Jankins' wife—not that he killed his own wife. Teddy was a slimeball who wanted to manipulate Teddy for his own profit and ease: Why else did he enthusiastically offer to pay for lunch & gladly takes Teddy's calls all the time?
