At this point it's been all four. There's the obvious injuries that will happen during sparring matches, there was Kitali and Estinien's fight from the ARR DRG quest, there's been numerous little hurts through figuring out relationships over the years, and the handful of Actual Arguments between Kitali and Aymeric.
Of the three of them, Estinien has done the most hurts, most of them unintentionally, but he's the leaverrrrr and he's not great at communicating with words and for Aymeric that was a problem when they were still figuring out where they stood with each other as a couple of young knights. Aymeric felt like for a long time he was just another body to Estinien and it took some very fraught conversations for Estinien to learn how to be better to him and for him. He cares, he truly deeply cares, he's just not great at really defining it and has to be asked directly to meet people where they are. He's getting better at it, though.
Early in their courtship Aymeric had asked Kitali if he was just a replacement for Estinien, which is rooted in his own self esteem issues, and Kitali turned the question around on him and he didn't quite know what to do with it. They were both still very much hurting at his departure, and in some ways they were filling a gap left behind, but that doesn't mean that they were replacing him.
There's Aymeric and Kitali's blowout argument about her not going to the peace summit with Varis and that ended in a very uneasy truce with the two of them keeping a distance from each other for several days.
