The words of "the bells tolled" point to the Battle of the Bells where he blames his defeat for Rhaegar's death. There is another layer as it could be referencing John Donne's sermon:

The death of Rhaegar, his only human connection meant that the bells tolled for Connington's humanity as well. He cut himself off, thinking if he had been the brutal Tywin Lannister he would have won the war and saved Rhaegar from an early death. He will do whatever he thinks he needs to do to seat Aegon on the throne to "atone" for his defeat. That includes possibly killing his kin and children.
