When I started Trambellir Sdn Bhd in Malaysia, the vision was clear, but the roadmap—less so. We weren’t trying to build a unicorn overnight. We wanted to create a dependable, human-centered digital platform that makes medical and wellness travel easier, safer, and more accessible.

Today, we’re operating in 35 cities across multiple countries, with partnerships ranging from boutique wellness retreats in Bali to urban diagnostic centers in Tokyo. It sounds smooth when written like that—but the reality of scaling across borders is anything but simple.

This article is less of a corporate update and more of a candid conversation—between me and you, the reader—about what it’s actually like behind the scenes. The hesitations, the pivots, and what we’ve learned along the way.

The early friction: trust isn’t automatic

When you’re expanding into a new country, the first assumption is that your digital model will scale cleanly.

It won’t.

Each market has its own language, regulation, health culture, and patient expectations. A service that feels premium in Malaysia may feel basic in Japan. What’s standard procedure in Thailand might be restricted in the UAE.

I remember pitching our model to a clinic in Tokyo—they stared at me politely and said, “You don’t understand how Japanese patients think about privacy.”

They were right. I didn’t—yet.

So I paused expansion in that market and spent three months just listening. That patience paid off. Today, Tokyo is one of our strongest cities in terms of user engagement and clinic re-bookings.

Lesson? Speed is seductive, but understanding is better.

Finding the right partners—not just any partners

In the early days, we thought onboarding more clinics quickly would create a stronger marketplace.

But it turns out, the wrong clinic can damage the entire brand. A single unclear pricing structure, a missed appointment, or a post-op complication—even if unrelated to us—can break trust.

So we slowed down. Our vetting process became strict. Some cities only have 3–5 listed providers. That’s intentional.

We’d rather show less choice with full confidence than a crowded page of unknowns.

The cultural nuance of wellness

“Wellness” isn’t a universal language.

In Japan, it might mean nutrition and low-stress environments. In Dubai, it may include advanced therapies and high-end experiences. In Indonesia, wellness often carries a spiritual or traditional component.

Our content team had to become anthropologists, translators, and cultural listeners. Not everything translates—and that’s okay.

Scaling isn’t about duplicating. It’s about adapting without losing your core.

Managing operations across time zones and temperaments

Thirty-five cities. Dozens of team members. Endless WhatsApp groups and Slack channels. Sometimes I wonder how we keep up.

Truth is, we mess up. We miscommunicate. We have different interpretations of “urgent.”

But we’ve learned to build systems that accommodate chaos, not avoid it. Flexible SOPs. Regional autonomy. A deep culture of documentation.

And yes—sometimes it just takes a midnight call to fix something. That’s part of it.

Keeping tech lean, but human-friendly

A lot of platforms chase automation. And yes, we have our share of booking engines, integrations, and dashboards.

But the truth is—our customers still want to talk to someone. Especially when they’re considering a health-related decision in a foreign country.

We invested early in real-time support, not just chatbots. It doesn’t scale perfectly. But it builds trust, and that’s harder to come by than clicks.

The Go Global Awards: a proud reminder

This year, Trambellir Sdn Bhd is proud to be a nominee for the 2025 Go Global Awards, taking place in London and hosted by the International Trade Council.

For me, it’s a reminder that the daily grind of scaling—the paperwork, the compliance, the local adaptation—leads somewhere. It leads to conversations that matter.

What’s next? Slower, smarter, stronger

We’re still scaling, but we’re more mindful now. Less obsessed with numbers, more focused on depth.

Some cities may not grow quickly. That’s okay. We’ll listen first.

Some partnerships may take a year to build. That’s okay, too.

Because when a traveler entrusts us with their health—or when a clinic allows us to represent their services—we owe them our full attention.

People sometimes ask, “What’s been the hardest part?”

It’s not funding, or tech, or marketing. It’s keeping the soul of the company intact while growing beyond what we imagined.

It’s hard to believe it’s been over a decade since we opened the doors to our very first Artesana factory in Tecuci. Time has moved quickly—but that little building, tucked away in the heart of Romania, holds the story of how it all began.

We didn’t start with much. A small team, a handful of machines, and a big dream: to make real dairy products the way they used to be made—by hand, with care, and with respect for tradition.

Here’s a look back at those early days—where the smell of warm milk and new paint filled the air, and every bottle we capped felt like a small victory.

The Dream That Became a Plan

Back in 2010, we saw something missing in the Romanian dairy market. There was plenty of milk—but most of it was ultra-processed, standardized, and stripped of flavor.

We wondered: what if we brought back real milk? What if people could taste cream rising to the top again? What if we honored simplicity—and let the product speak for itself?

That idea became the seed for Artesana. And two years later, it became a working factory.

Building with Intention

The factory wasn’t big. But every part of it was designed with one goal: preserve the integrity of milk.

We installed a low-temperature pasteurizer (when most advised us not to). We sourced glass bottles instead of plastic. We refused to homogenize the milk, knowing full well that it might confuse customers used to uniform white liquid.

And we took our time.

Those first batches were small. Filled by hand. Labeled one by one. Sometimes we stayed past midnight, waiting for jars to cool just right.

But when we tasted the result—thick, rich, tangy, alive—we knew it was worth it.

First Deliveries, First Feedback

Our first delivery route was barely 50 kilometers. A few local shops in Galați and Brăila agreed to take a chance on us.

We’ll never forget that first call:

“Hello? I think something’s wrong. The milk has... a layer on top.”

We smiled. “That’s cream,” we said. “That’s how it’s supposed to be.”

Over time, those calls turned into compliments. Then into loyal customers. Then into families who would visit our stall at fairs just to say thank you.

Growing Without Changing

Today, Artesana has grown. We’ve expanded our range. We’ve added yogurt, Sana, kefir, cheese. We’ve built a second factory.

But that original facility in Tecuci? It’s still running. Still filling glass bottles. Still doing things slowly, intentionally, the way we always meant to.

We walk through it sometimes just to remember how far we’ve come—and how little we’ve compromised.

What Hasn’t Changed

We still source from small Romanian farms.

We still pasteurize gently.

We still use glass.

We still believe that food made with care tastes better.

And perhaps most importantly: we still believe in earning trust one jar at a time.

From this strategic hub, cargo flows into the landlocked interiors of Burkina Faso, Mali, and Niger, and outward to Europe, Asia, and beyond. With its latest expansion, PortLink isn’t just adding square footage. It’s expanding possibilities.

At Indelox Service Private Limited in India, we’ve had the privilege of partnering with PortLink on several cross-border initiatives. What we’ve consistently observed is that their approach to infrastructure is never about size for size’s sake. It’s about solving actual pain points. And this new facility in Tema is a prime example of that principle in action.

Let’s break down what makes this facility a game-changer—not just for PortLink, but for every importer, exporter, and 3PL operating in the region.

Location, Location—and Precision

Strategically positioned just minutes from the main Tema Port terminal, the new PortLink facility significantly reduces drayage time and port congestion delays. Containers that once took 6–8 hours to reach a warehouse can now be unloaded within 90 minutes of release.

That time savings? It adds up quickly.

We recently coordinated a multi-container shipment of critical health supplies from India to Ghana. With PortLink’s new facility as the reception point, we shaved a full day off the distribution timeline. For temperature-sensitive goods and just-in-time supply models, that difference is everything.

Purpose-Built Design, Not Retrofitted

Unlike many logistics facilities that are adapted over time, PortLink’s Tema site was designed from scratch with one objective: service quality.

Separate zones for ambient, chilled, and frozen storage. A dedicated customs inspection bay with pre-clearance lanes. CCTV-monitored loading docks. And perhaps most importantly—redundant power systems to support reefer plug points and temperature control during outages.

PortLink has equipped the Tema facility with an integrated Warehouse Management System (WMS) that syncs in real time with its transport management platform and customs interface. For clients, this means end-to-end visibility—from container arrival to delivery at final mile.

At Indelox, we’ve seen this benefit firsthand. In a recent pharma shipment, our teams in India were able to monitor storage conditions, clearance progress, and dispatch readiness through shared dashboards. No email chains. No ambiguity. Just clarity.

At Nutritech Nutrition, based in Pakistan, we don’t always talk about what happens behind the scenes. But every now and then, we pause. We look back. And we realize: our work isn’t just formulas and logistics. It’s also the people, the places, and the conversations that shape our purpose.

So for this post, I’m not writing to explain a product or explore a nutritional concept. I’m simply sharing highlights—from events that moved us, challenged us, and reminded us why we do what we do.

📍 Regional Health Symposium – Lahore

In March this year, we were invited to participate in a multi-day symposium focused on non-communicable diseases and nutritional interventions. Our medical affairs team presented findings on the clinical use of Energid Plus in post-COVID recovery patients.

But more than that, we listened. To dietitians from across the country sharing case studies. To young medical students asking tough questions about supplement ethics. To government officials looking for scalable solutions.

There was energy in the air—something that’s hard to quantify, but you feel it when you’re there. You see it in the photos: a crowded booth, hands reaching for samples, eyes locked on a data slide. That’s where progress lives.

📍 Pediatric Nutrition Drive – Interior Sindh

This was less formal. No media. No stage. Just us and a group of local health workers visiting clinics and schools in underserved areas. We brought boxes of infant and toddler nutrition products, provided caregiver training, and most importantly—we listened.

📍 Hospital Nutritionist Roundtable – Islamabad

We hosted this event ourselves—something we don’t do often. A small roundtable, limited to 15 hospital-based nutritionists and pharmacists. No keynote speakers, no spotlight. Just discussion.

And the outcome? A shared framework for standardizing nutrition protocols in post-surgical care—a direct result of field feedback on our Enerbest and Glumin SR products.

Earlier this year, we finalized an export partnership with a leading East African distributor. It wasn’t flashy, but for us, it marked a new chapter. Because when Nutritech’s products reach clinics in Nairobi, Kampala, or Dar es Salaam, they carry not just nutritional value—but the ethics, care, and quality control that define our work here in Pakistan.

This one hasn’t happened yet, but we’re already gearing up for it.

Step 2: Booking & Preparation

• Route Confirmation & Booking – Once we receive approval, we confirm carriers: airlines, liners, vessel slots, and trucking plans. • Documentation Prep – Begins here: commercial invoice, packing list, HS classification, certificate of origin, special permits (e.g. phytosanitary, export).

Step 3: Cargo Consolidation & Pickup

• Pickup Coordination – We organise haulage to collect cargo—ensuring correct vehicle types (e.g. reefers, flatbeds). • Consolidation (optional) – Where multiple clients’ cargo share containers/trucks, we load, rubber-band seal, and document contents carefully.

Step 4: Origin Handling & Export Clearance

• Loading at Origin – Cargo is offloaded into an airport or seaport terminal via certified handlers. • Export Customs Clearance – Pre‑arrival documentation, submission to Ghana Revenue Authority; any inspections handled before shipment departs.

Step 5: Main International Leg

• Sea Freight – For ocean routes, containers are booked and loaded, moving along established maritime lines. • Air Freight – For time-sensitive or high-value shipments, cargo is palletised and flown via cargo or passenger aircraft. • Freight Tracking – En-route, GPS updates track vessels/trucks and airline confirmations.

Step 6: Destination Customs & Import Handling

• Arrival Notification – We notify clients when cargo enters the destination country (airport or port). • Customs Brokerage – Licensed brokers handle import duties, tariffs, inspections, and secure release documents. • Inspection & Warehousing – If flagged, cargo undergoes inspection; otherwise, it’s cleared directly to warehouse.

Step 7: Last-Mile Haulage & Delivery

• Pickup from Terminal – RFS organises haulage aligned with clearance notice and client timing. • Safety Checks & Route Planning – We confirm truck readiness, permits, route suitability, and ETA. • Final Delivery – Cargo arrives at consignee’s site. We collect proof of delivery signatures and close digital updates.

Step 8: Post-Delivery & Follow-Up

• Invoicing & Accounting – We issue clear invoices with line-item breakdowns: freight, customs, haulage, etc. • Documentation Hand-Off – All paperwork (BL/AWB, customs docs, insurance, delivery POD) is shared digitally. • Debrief & Feedback – We meet internally—and with clients—to review performance, identify improvements, and reinforce learning.

It’s not just about nice photos or a well-designed booking platform. When a traveler decides to fly across borders for a health-related procedure—whether it’s a cosmetic tweak, a diagnostic test, or a wellness retreat—they’re making a decision that requires trust. And trust, in this business, must be earned.

But here’s how we make it work.

For every country we operate in, we work with local health consultants or legal advisors who understand the regulatory landscape. They help us:

Verify clinic licenses

Confirm practitioner certifications

Flag red zones (e.g., restricted procedures, banned substances)

Understand new or evolving regulations (especially post-pandemic)

This “on-the-ground” knowledge is non-negotiable. We’ve turned down partnerships before simply because we couldn’t verify a clinic’s licensing despite multiple attempts.

That’s a line we don’t cross.

2. Annual provider re-verification

Medical licenses can expire. Clinics can change management. Regulations shift. So we conduct a yearly re-verification audit of every provider on our platform.

If they can’t provide updated documentation—or if something feels off—we pause or remove them. No exceptions.

It’s a lot of paperwork. But honestly, the peace of mind it offers our users (and ourselves) is worth it.

3. Transparency over perfection

Not every clinic is perfect. But we don’t hide behind branding.

If a provider doesn’t offer English-speaking staff? We say it clearly. If recovery periods are longer than average? We spell it out. If a service is still considered “experimental” in certain regions? We include disclaimers and recommend travelers consult their own doctors first.

Our goal isn’t to curate a flawless image. It’s to give people the full picture, so they can make informed decisions.

Is this legal in the location it’s being offered?

Is the provider operating within scope (e.g., no general clinic offering surgical services without appropriate facility designation)?

Are treatment claims medically sound, or dangerously exaggerated?

If the answer to any of those questions is “no” or “unclear,” we do not publish the listing.

5. User feedback loop

This might be the most powerful tool of all.

We encourage all travelers to leave reviews—not just star ratings, but detailed feedback. And we read every single one.

If something unexpected or concerning happens during treatment, we reach out to the clinic and ask for clarification. In serious cases, we involve local authorities or remove the listing entirely.

Travelers are our eyes and ears in the real world. Their stories sharpen our compliance efforts more than any policy manual ever could.

Real example: handling cross-border concerns

A few months ago, we were approached by a clinic in Southeast Asia offering a new “wellness enhancement” drip with ingredients that sounded... vaguely pharmaceutical.

We consulted a regional medical officer, checked labeling standards, and reached out to two national agencies. Turns out, one of the ingredients wasn’t approved for non-hospital use.

We thanked the clinic, explained the situation, and declined to onboard them.

Could we have just quietly listed it and let the market decide? Maybe. But that’s not what we’re building.
