Fluids and Electrolytes: Fluid Volume Excess
What is it?
Fluid volume excess, or fluid volume overload, is caused by an excess intake or retention of fluids. This can be caused by a couple of things, mainly heart failure, chronic kidney disease, and excess of fluid intake (orally or IV). Â
I would like to take this moment to say that you should always monitor your IV site and flow rate. You, as the nurse, can overload your patient.Â
Manifestations:
So, when your body has excess fluids, your body’s mechanisms are overrode. Your pulse becomes bounding and forceful. The heart rate stays increased (tachycardia), to try to rid all the excess fluid. Because of the amount of fluids in the body, blood pressure is going to be high (hypertension). Of course, fluid retention is going to cause weight gain. If you remember our cardiac review, when fluid can’t be excreted enough, fluid backs into the lungs. This causes crackles in the lungs and edema.Â
Lab Changes:
Lab results are actually what diagnoses these fluid problems. Since there is an excess of fluids in the body, it dilutes all the results. So we see what’s called hemodilution. This is going to show in a decreased H&H and electrolytes.Â
Treatment:
Priority always goes to airway and breathing! So, since there are crackles in the lungs, we need to monitor lung sounds and oxygenation every 2 hours. We need to be watching to see that they are urinating.Â
We give these patient’s diuretics to help them pee it out! Diuretics are amazing. If you’ve been in a clinical setting yet, hopefully you’ve seen the effect of diuretics. They will make you pee and it really does release the fluids. While they’re on diuretics, they are at a risk for more electrolyte imbalances, specifically potassium. They are at a risk for falls, also.Â
We are also going to restrict these fluids until we get lab results back down to normal.Â
