What red light therapy actually is
The scientific name is photobiomodulation, formally added to the National Library of Medicine's Medical Subject Headings in 2015. Older clinical papers and device clearances call it low-level laser therapy, or LLLT.
It is not a heat lamp and not tanning. The light is non-ionizing and, at therapeutic intensity, non-thermal. What makes it work is a specific interaction between certain wavelengths and a specific enzyme inside your cells.
The mechanism: one enzyme, one displaced molecule
Mitochondria produce ATP, the energy currency your body runs on. The final step is handled by cytochrome c oxidase — the terminal enzyme of the electron transport chain, containing four light-absorbing metal centres: two heme groups and two copper centres.
Those metal centres explain why particular wavelengths work. Tiina Karu's foundational research showed that the wavelengths most strongly stimulating energy-dependent cellular functions correspond to the absorption bands of these copper centres, with peaks near 620, 680, 760 and 820 nanometres. Delpy and Cope demonstrated that more than 50% of light absorption between 800 and 850 nm is attributable to cytochrome c oxidase, with haemoglobin playing a minor role.
Here is what matters for a man over 40. Nitric oxide competitively binds to cytochrome c oxidase and inhibits it — occupying the site where oxygen should go. Under chronic stress, inflammation or ageing, nitric oxide accumulates, the enzyme slows, and cells produce less energy than they should. Red and near-infrared photons displace that inhibitory nitric oxide, allowing respiration to resume.
ATP production rises. Downstream, reactive oxygen species and calcium act as signalling molecules, activating pathways including PI3K/Akt that govern proliferation and repair — a cascade reviewed in detail in the Journal of the American Academy of Dermatology.
Wavelength determines depth, and depth determines the result
This is where most men waste money. Wavelength is a physical parameter deciding how deep photons travel and therefore which cells receive them.
630 nm — roughly 1–2 mm. Epidermis and upper dermis. Fine for surface tone, largely useless for structural collagen.
660 nm — reticular dermis, 2–4 mm. Where the fibroblasts producing collagen and elastin actually live.
650–655 nm — the band used in FDA-cleared hair devices.
810 and 850 nm — near-infrared, reaching muscle, joint and connective tissue.
980 nm — works through water absorption, a different mechanism entirely. Not interchangeable with 810 nm.
The practical rule: 660 nm and 850 nm. Red for skin and follicle, near-infrared for muscle and joint. Devices advertising eight or ten wavelengths dilute the irradiance available at the two with evidence behind them.

Dose matters more than duration
Photobiomodulation follows the Arndt-Schultz biphasic principle: too little produces nothing, moderate produces benefit, too much becomes inhibitory. The reactive oxygen species that signal helpfully at moderate dose become oxidative stress at high dose.
If you are not getting results, the answer is almost never more. Ten to fifteen minutes at a sensible distance, three to five times weekly, is a defensible starting point.
What the evidence actually supports
Skin — strong. Wunsch and Matuschka ran a randomized controlled trial with 136 volunteers, treating twice weekly at approximately 9 J/cm² for 30 sessions. Using ultrasonography, blinded photography and digital profilometry, they documented significantly increased intradermal collagen density and measurable reductions in wrinkle severity and skin roughness — persisting at six-month follow-up. Barolet and colleagues confirmed the collagen mechanism using a pulsed 660 nm source.
Hair — strong. Two parallel randomized, double-blind, sham-controlled trials using a 655 nm device showed significant increases in terminal hair density at 26 weeks. A real-world study of 1,383 patients using an FDA-cleared helmet found roughly 80% clinical effectiveness, with better outcomes past one year of use.
Recovery — good, with a twist. Red and near-infrared at 660 and 850 nm reduce post-exercise creatine kinase and lactate dehydrogenase. A 2024 meta-analysis of 34 randomized controlled trials found pre-exercise photobiomodulation significantly improved muscle endurance and recovery. Maximal strength — not proven. A meta-analysis in older adults found no significant improvement in maximal strength when photobiomodulation was added to resistance training.
Combining it correctly
The most common error is stacking every recovery tool into the same post-training hour. Cold exposure blunts the inflammatory signalling that drives training adaptation — separate cold from lifting by several hours. Sauna and red light are complementary. Red and near-infrared carry no circadian-disrupting blue content, so evening use is generally sleep-compatible. And repair requires substrate: 30 to 40 grams of protein per meal.
Where it belongs in your program
Red light therapy is one of the better-supported items in the modern longevity toolkit. It is also, unambiguously, an adjunct. It improves mitochondrial energetics in tissue already being asked to do something. It supports repair from a training stimulus you have to supply.
