This was so hard for me to decide, but I think Azaela's fatal flaw is that she can't separate being good from being useful.

She genuinely wants to help people. She wants to believe that if she works hard enough, she can make things better, and while that, on its surface, is not a bad goal to strive for, it also makes her very easy for Shinra to exploit.

Azaela will stay in situations longer than she should if she can convince herself that some good will come out of it.
