there's a parallel between women not being allowed to be priests, Cardinal Lomeli being manager instead of shepherd, playing role of staying neutral, making the Conclave run smoothly, preferably not getting into the spotlight just as nuns. There's also ultra radical (lmao) idea (brought by the late Pope and presumably Bellini) of allowing women to hold administrative roles in Curia

there are lines you can interpret as Lomeli having dysphoria (disclaimer: likely intended to be his insecurity and depression caused by cancer, old age etc)

but also there's some envy in a way he thinks about other characters performative masculity, something in a lines of "you are pretending just like me, but why are you not suffering from it?" (again, same disclaimer as w/ dysphoria)
