[5e did the bulezau dirty. Like the armanite, the bulezau first appeared in the Planescape Planes of Chaos boxset back in 2e. There, it was a nine foot tall bruiser. Possibly to minimize confusion with the (also minotaur-like) goristo, in Mordenkainen’s Tome of Foes the bulezau is now much smaller and weaker. They’ve basically turned into the schirs from Pathfinder. This version reconciles the size of the original with some of the abilities of the 5e version. My conception with the flavor text is that these would fit in perfectly to the Wrath of the Righteous AP. ]

CR 9 CE Outsider (extraplanar)
This tall, rangy humanoid has the head of an ungulate with long curving horns. Its tail is long and thin, ending in a cluster of spikes. Scabs and sores pock its skin, and a cloud of flies buzzes around it. It holds a great polearm in its clawed hands.
The first bulezau were created as a collaboration between the demon lords Deskari and Baphomet. Now the souls of monstrous humanoids truly devoted to chaos and evil may spawn a bulezau. Combining the deadly powers of bestial strength and swarming insects, they are truly a horror to behold on the battlefield. Bulezau are difficult to control and devoted to violence at all costs, and they are liable to turn on their allies if not given sufficient outlets for their rage.
A bulezau typically enters combat with a charge, lowering its head to gore with its mighty horns. Enemies that get too close are punished with the biting flies that constantly emerge from their rotting flesh. Their sting carries the threat of demon fever, and even those who escape a bulezau on the battlefield may succumb to its disease in the days to come. Although bulezau have several potent spell-like abilities, they rarely use them unless ordered to by a more intelligent commander. Bulezau typically fight to the death.
Because of their status as relative newcomers to the armies of the Abyss, bulezau have a perpetual chip on their shoulders. Vrocks and armanites are frequent targets for competition, and most bulezau enjoy pushing around weaker demons like dretches, schir and brimoraks. Different bulezau have the heads of different animals, with goats, bulls and antelope being the most common, but any horned mammal is possible.

Cloud of Flies (Su) A buzzing cloud of flies surrounds a bulezau, granting it a 20% miss chance against all attacks. Creatures adjacent to a bulezau take 1d6 points of piercing damage that overcomes DR as an evil weapon, and must succeed a DC 21 Fortitude save or be sickened for 1d4+1 rounds. A bulezau that takes 10 or more points of damage from an area of effect spell or ability loses its cloud of flies for 1d4 rounds. The save DC is Constitution based.
Disease (Su) Demon fever; sting—injury; save Fort DC 21; onset immediate; frequency 1/day; effect 1d6 Con damage; target must save a second time or 1 point of damage is drain instead; cure 2 consecutive saves. The save DC is Constitution based.
Slippery Mind (Ex) If a bulezau is affected by an enchantment spell or effect and fails its saving throw, it may attempt a second saving throw against the same DC 1 round later. If it fails this second save, it cannot make additional saves against this effect.
