The fact that the person shown under the "save me" is Akutagawa, who needs saving both in the literal (he's a vampire atm) and metaphorical sense. What's more, in that scene he's fighting with Atsushi, a person who's supposed to help him save himself.
For me this is like Akutagawa saying "save me" to Atsushi. (But also the fact that he's the one who saved Atsushi... They really do need each other).

been thinking about how after atsushi yelled at akutagawa that one can’t possibly live without being told it’s okay to do so, he later angrily thought that it was not something he needed to be told.
now atsushi is screaming at him, desperate for him to wake up, and trying so hard not to hurt him. akutagawa saved atsushi’s life, and wordlessly told him that it’s okay for him to live. and now, though his actions, atsushi is trying to tell akutagawa that it’s okay for him to live. because no one has ever told him so.
