While Bellatrix got the Black Family Madness, and protected her sisters even though they hated her, she let Andromeda run away, and when Andromeda ran away, Narcissa changed her mind on Death Eaters too, staying in the family, but distancing herself from the Dark Magic. Bellatrix had used her power as an older sibling and it had worked, because her whole family was in Slytherin. Sirius couldn’t change Regulus’s fate from the moment he was sorted in Gryffindor, because his whole family would be against him, while Bellatrix’s whole family was with her.
