Devil is a Part-Timer headcannon because I need motivation...
I think about this from time to time. Basically Urushihara is about 11/ 12 instead of 18. He's still little and has piercings just like the anime/ manga. Anyway, he has a bad cough for a few days; it's been rainy and cold lately in Japan, and either Emi or the landlord makes a comment that Maou should take the boy to a doctor or at least get him some medicine. Who knew being a human could be so hard, right? Urushihara insists he's fine; he's not a kid - he's the great demon lord, Lucifer, and he doesn't need to be babied.
A few days go by, and Urushihara shows up at Maou's work. He's been pretty mellow the past few days, keeping his grumpy attitude to a minimum and actually listening to Ashiya for once. The boy explains that he and Ashiya were out shopping for groceries when they got into a heated argument before Urushihara left, saying he'd find his own way home (he didn't have time to be treated like a kid). Ashiya of course calls Maou's work frantic, saying he lost Lucifer, how he was such a brat, yada yada. Maou explains the kid was at his work and instead of coming to get him, he would just walk him home after his shift ended in a few hours due to the storm.
During the next few hours, Urushihara has been pretty quiet. He mostly sits quietly at a table near the door, playing on his gameboy (he's been coughing up a storm before Chiho asks politely if he could put on a mask because he was scaring off customers - he puts on a mask). Other than that, he's been pretty chill/ out of character. The manager asks Maou if his little brother (Urushihara) felt alright due to the kid barely touching the free fries she gave him and the soda. Maou talks her down, but keeps his eyes on the kid when not serving customers. An hour before his shift ends, the manager says Maou could go home due to the storm keeping people away, and the fact that the poor kid looked like hell - Urushihara fell asleep at the table about an hour ago.
Maou leaves his bike and decides to walk home with the kid (in the rain). Urushihara is shivering due to his lack of jacket, and Maou sighs, "you need to be more careful," before pulling his McRonald's jacket off and handing it to the kid (who in turn says he doesn't need it, but takes it anyway). The jacket was big on Maou but it's huge on Urushihara as the kid shoves his gameboy in the pocket. At this point, Maou can definitely tell something is wrong with the boy; he keeps running into people, his gaze is far away and distant, he's stumbling, etc. Right before they reach the train station, Maou realizes that Urushihara is quite a ways behind him.
Urushihara is burning, sweat clinging uncomfortably behind his mask as he tries breathing through the thin cotton. The jacket around him felt nice and warm but only added further heat to his body as cold winter air nipped at his face. He stumbles, falling forward as someone shoves him, his hands and knees colliding harshly with the pavement as he sucks in a shallow breath. His stomach hitches as he shuts his eyes, the cement spinning, footsteps of people rushing past him echoing loudly in his ears as he bites back a violent gag.
Gentle fingers brush behind his ear, pulling the mask from his face as his stomach heaves again and what little he'd eaten over the past few day's splatters against the ground in sickening waves. He retches again, a hand on his back, another on his shoulder, keeping him upright as he tilts slightly. By the time he's finished, he's more miserable than before, sweat paints his face, he's shivering, and his hands feel gross and sticky.
Maou basically comforts the boy, testing his fever, and trying to clean his hands off before pulling Urushihara up into his arms. The boy's head smacks against his shoulder as he mumbles, "I can walk..."
I have this idea of Maou carrying Urushihara the rest of the way; standing with him on the train (there is nowhere to sit down), holding him gently against his chest like he would Alas Ramus, rubbing his back and stuff. The kid looks tiny in Maou's arms, especially with his jacket swallowing his small frame. An older woman taps Maou's shoulder at one of the stops, asking if this was "his son's," holding up the gameboy that was in the boy's pocket (it slipped out during one of the stops). Maou thanks her before exiting the train.
Maou waits at a light after leaving the station (he left his bike at work due to the storm and Urushihara being here - two people riding a bike in Japan is illegal - a lesson Maou has learned many times). While waiting for the light, Maou hears, "Papa! Papa!" and hears Emi yelling, "Alas Ramus! Stay close! Don't run off!" before feeling a smack against his legs. He glances down to see Alas Ramus (idk she's like 4), and he smiles, patting her head as Emi runs up behind her, an umbrella in hand.
They exchange looks before Emi crosses her arms, asking what Maou was doing here before noticing Urushihara in his arms. At this point, Urushihara is either out of it or asleep, his cheek pressed against Maou's shoulder, sweat and rain present on his face, his cheeks and the bridge of his nose, red, his breathing uneven; he has one arm swung around Maou's neck, the other hanging loosely at his side. Obviously anyone can see the kid is sick.
Alas Ramus asks what happened to Luci's knees - they're bleeding, scratches from the pavement present as dried blood followed the path on his shin. She also asks to be picked up, but Maou explains, "Papa can't hold you right now, Princess."
They walk towards the apartments together - Emi and Suzuno wanted to have a girl's night and said Alas Ramus could join as well. While walking back, Emi takes note on how small Urushihara looks, how bad his cough is and suggests they stop at a drugstore for cough medicine. They stop, Alas Ramus running to the toy section. Maou stands in front of the children's medicine for a solid ten minutes, reading labels. He's unsure what to get (also hearing Ashiya in his head stating they can't afford anything fancy - stuff like that). Urushihara is older than the medicine describes, but smaller in weight that the limit. An older man, the pharmacist asks (again) if the gameboy that fell from the jacket pocket was Maou's son. Maou puts it in his back pocket instead of the jacket as Urushihara moans slightly, Maou shifting him from one side to the other. The pharmacist laughs softly, saying him, Urushihara, Emi and Alas make a cute family. He also suggests some meds.
Emi's voice is loud as Alas starts scream crying. She wants a sword so she can be like her Mama, but she also wants the doctor kit so she can help Luci feel better. Her loud wails draw Maou over and Urushihara starts whimpering, his eyes opening slightly as his stomach hitches and he gags roughly. Maou and Emi pled with Alas to lower her voice as Maou lowers himself slightly, pressing the boy closer to him, his hand rubbing gentle circles in his back as he whispers to him, hoping the boy doesn't throw up. Urushihara presses his forehead harder against Maou's neck, swallowing several times as Alas's cries die down and the boy relaxes slightly. Basically, I want Alas to throw a fit, and Maou trying to calm Urushihara and Alas down.
They buy the meds and walk back to the apartment as the storm picks up. Ayshia starts to complain about Urushihara before Maou shuts him down, and then the blonde-haired man starts to worry, fretting over the kid. Alas came over to put kitty bandaids on Urushihara's knees, taking his temperature (little kid stuff) before Maou encourages the kid to drink something (Ayshia tries to get him to eat something with not much luck), and the night goes on. Alas Ramus, Emi and Suzuno all end up sleeping at Suzuno's due to the weather. In the middle of the night, urushihara's cough gets worse (he's coughing up phlegm, vomiting and stuff) and his fever spikes. He starts getting delirious -crying and acting like a kid (his age); pleading for Maou and Ashiya not to kick him out (he's already been kicked out of heaven, and even Alba didn't want him, etc, etc - he's got baggage). Maou or Ashiya, in an attempt to bring his fever down and to get the kid to stop crying, throw him in the shower - or take him to the bath house - either they have shower in this fic or they don't, dealer's choice. Basically, one of them hold's a crying Urushihara under or in the water until his fever starts to drop.
All-in-all, it's a long night. Idk this has been on my mind for a while so I'm glad I finally wrote out the headcannon. Lmk thoughts.
