I honestly don't think that anyone but conservatives, fundies, and the criminally old think trans genders are less "real" than our binary biological genders.

I won't get into non-straight sexuality, because that's a whole other can of worms I could just type about for hours.

Regarding coming out, it's a backlash regarding perceived difference in a community.  Let's just assume that MOST reasonable families these days don't really care who their kid is interested in, as long as they don't end up hurt, abused, or with an STD/STI.  But the fear is still there, since people identifying as gay/bi/trans/asexual and so forth have been treated poorly for so long and so recently in our history.  And so, coming out is still thought to be required, and it will be for as long as most straight people are complete dicks about it.

The same thought can apply to trans individuals.  Any parent who truly gives a damn about their kid will honor their wishes to be a different gender.

What I'm saying, though, is that if you were born male and you identify as male, you are male.  If you were born female, and you become male later in life, then you deserve the respect of being called male.  If you feel you identify with both or neither gender, then that's okay, too.
